All returning and incoming student athletes at Carthage College must complete all forms listed and follow these instructions exactly as stated. Do not send any printed forms by mail or give to coaches. All printed forms should be uploaded as instructed, not given to coaches or mailed in.

Medical Participation Forms and Pre-Participation Checklist

This medical form and this pre-participation checklist are both required to be filled out by all student athletes.

LOG-IN INSTRUCTIONS

Your username will be the first part of your Carthage email address. Your password consists of the first initial of your first name and the first initial of your last name, and your Carthage College ID number. For example, John Smith has an email address of jsmith2@carthage.edu and his Carthage College ID number is 0000000. His username will be jsmith2 and his password will be js0000000. If you are having problems logging in, please contact the helpdesk at (262) 551-5950.

NCAA Student Athlete Information and Education

ATHLETES MUST HAVE PRIMARY INSURANCE COVERAGE PRIOR TO PARTICIPATION

The policy must:

Cover intercollegiate athletics
Be applicable to coverage in Wisconsin
Be retained for the length of athletic participation (including pre-season and nontraditional seasons)
